Free cookie order form template for custom, decorated and Christmas cookies

A cookie order looks simple until the questions start. How many dozen, which shapes, what colors, bagged or boxed, and is the name spelled with one n or two? The cookie order form below asks all of it once, puts the price per dozen beside each line, and gets the deposit on paper before you mix the icing. Print it, copy it, or download it as a CSV.

Key takeaways
  • A printable cookie order form for custom and decorated sets: flavors, dozens, design, colors, packaging, pickup and deposit.
  • A cookie price list template you can fill in and post.
  • A Christmas cookie order plan for 2026, worked back from Christmas Eve.
  • What changes the price of a cookie order, with two worked examples.

One customer per sheet. Each cookie line carries the flavor or shape, the number of dozens and the price per dozen, so the total adds up in front of the customer instead of at pickup.

Write your minimum order (one dozen per design is common) and how much notice you need at the top of the sheet.

Two dozen cookies can be a $69 order or a $147 one. The flour barely changes; the hours and the packaging do. Ask about each of these on the form, because each one belongs in the price.

Two examples using the bakery pricing calculator math (ingredients, packaging and hours at $20, then 10% overhead and a 25% margin): a mixed Christmas box (four dozen gingerbread, snowballs and shortbread, packed by the dozen) comes to $34.50 a box, and a dozen decorated sugar cookies with royal icing to $73.50. Your costs will be different, so run your own numbers.

Customers order faster when the prices are already public. Fill in your cookies, the unit you sell them by and your price, then post the list on your shop, your stories and the market table. The first two rows are the worked examples above.

Add a line for your minimum order and your packaging options, so nobody asks for one decorated cookie in a gift tin.

December cookie orders arrive all at once: cookie exchanges, office parties, teacher gifts and the Christmas table. Christmas 2026 is on a Friday, so most pickups land between Saturday the 19th and Wednesday the 23rd. Here is that run worked backwards, with decorated sets started first because royal icing needs a night to dry.

  • Cutoff: a date and an hour, like “Christmas orders close Sunday, December 13 at 8 pm”.
  • Limit: the number of boxes you can decorate and pack in the days you have.
  • Menu: three or four box types beat a build-your-own box. Every option is another line on the bake list.
  • Deposit: half up front for custom sets, or full payment for standard boxes.

Selling cookies from a home kitchen

Cookies are among the foods cottage food laws most often allow, because they keep without a fridge. Fillings and frostings that need one, like cream cheese frosting, custard or whipped cream, usually do not qualify. Most states also ask for a label on each package.

The cottage food laws by state page lists what each state allows and its sales limit, and the label maker prints the label your state asks for.

A form still makes the customer write everything down and leaves you adding the totals. A shop link turns it around: your cookie boxes sit there with photos and prices, customers tap what they want and answer your questions, and the order arrives already added up.

In Confita, a Christmas cookie drop gets its own cutoff, pickup windows and a limit on each box, so the 41st box simply cannot be ordered. Custom sets can go through a quote with a deposit instead. It is US$9.99/month after a 7-day free trial.

Questions, answered

Frequently asked questions

What should a cookie order form include?

The customer’s name and phone, the pickup or delivery day, the occasion, one line per cookie with flavor or shape, dozens and price, the design and colors, any names or text, the packaging, the subtotal, the deposit and the balance due, the payment method, and an allergy line. The form at the top of this page has all of it.

Is there a free printable cookie order form?

Yes, it is on this page with no signup. Print it, copy it as text or download it as a CSV. For a PDF, open your browser’s print option (Ctrl + P or Cmd + P on a computer, or Share and then Print on a phone) and choose “Save as PDF”. The page prints only the sheets.

Can I turn this into a Google Form?

Yes. Use “Copy as text” on the form, then add each line as a question in Google Forms: short answer for names and dates, a multiple choice for packaging, and a paragraph for the design. Keep the deposit and balance lines on your side, since a form cannot take the payment.

How much should I charge for a dozen decorated cookies?

Price it from your own batch: ingredients, packaging, hours, overhead and margin. In the worked example on this page, a dozen decorated sugar cookies comes to $73.50 and a mixed Christmas box of 12 to $34.50. The decorating hours make the difference. The bakery pricing calculator does the math with your numbers.

How far ahead should customers order custom cookies?

Ask for at least a week for standard cookies and two weeks for decorated sets, longer for large orders. In December, replace the notice period with a fixed cutoff date, because every order wants the same week.

Should I take a deposit on cookie orders?

For custom and decorated sets, yes. Half up front with the balance at pickup is common, and it covers ingredients and packaging bought for that order if the customer cancels. Standard boxes are often paid in full when ordered.

Can I use this as a Christmas cookie order form?

Yes. Add your box types as cookie lines, put the Christmas cutoff and pickup windows at the top, and use the price list for the menu post. The Christmas section on this page has a 2026 plan worked back from Christmas Eve.
